Day Program

Located on the Tri-C Western Campus in Parma, Ohio, our Creative Living Transitions comprehensive day program is one of three Creative Living Day Services we offer.

Components of this program include:

  • Instruction in executive functioning, social thinking and reframing:
  • Communication
  • Social engagement
  • Management of stress
  • Organization, time management and planning
  • Emotional control
  • Adaptability to changing conditions
  • Relationship building
  • Prioritizing and initiating tasks
  • Health, nutrition and wellness classes
  • Life skills education and training
  • Individualized physical fitness program
Additional Day Services

Post-Secondary College Program

As a component of our Creative Transitions day program, an individual can elect to participate in college courses to earn a post-secondary certificate or college degree at Tri-C Western Campus in Parma, Ohio. To participate, an individual must pass a placement test. We offer support, if needed, for preparing for the placement test.

Medina Creative Housing provides the following academic supports within this program:

  • Coordinating tutoring service from the educational institution
  • Round trip transportation for both residential and commuter students
  • Academic supports to assist students in making a successful integration into post-secondary education

Career Skills & Exploration - Summer Program

The goal of this program is to develop job skills in real life, community based business settings as a stepping stone to permanent employment. Support and guidance will be provided either directly or intermittently by a job site supervisor. A weekly academic supervisor will meet with each student and provide a job review/evaluation of goals set by the student and academic advisor.

Components of this program include:

  • Career exploration and transition counseling
  • Real life job experiences
  • Job coaching and shadowing
  • On site observation and supervision
  • Developing communication skills in the workplace
  • Career skills necessary for a successful transition into a work setting
  • Developing conversational skills necessary for transitioning into a post-secondary or workplace environment
  • Resume writing strategies
  • Instruction in banking, budgeting, and money management
  • Friday career exploration outing in the community
  • Transportation provided for both residential and commuter students

Students are assigned several different paid positions at job sites during the 7-week course. They work Monday thru Thursday from 9:00 am - 2:00 pm.

Students will participate in executive functioning and communication skills on Fridays from 2:00 pm to 4:00 pm.

Independent Living Experience

Medina Creative Housing owns an independent living experience home in Brunswick, Ohio with eight efficiency units, that is part of our Creative Living Transitions program. Individuals living in this home setting work on skills and goals in order to achieve independent living in a single family setting.

The independent living experience includes:

  • Individual efficiency units
  • Life skills strategies and independent living assistance
  • Appropriate social interactions and guided activities
  • Structured recreational activities and cultural experiences
  • All activities and recreational opportunities conveniently located within walking distance of Transitions house
  • Residential housing, with eight efficiency units, located in Brunswick, Ohio
